Urine high-risk human papillomavirus testing as an alternative to routine cervical screening: A comparative diagnostic accuracy study of two urine collection devices using a randomised study design trial

This randomised sub-study found return rates of 10.4% for Colli-Pee, 10.1% for FLOQSwabs and 7.4% for Evalyn-Brush. All devices yielded sufficient DNA with low invalid rates, but the authors noted the study was limited by low overall participation.

In 498 women living with HIV, self-collected first-void urine and vaginal samples showed 78% high-risk HPV prevalence and good genotype concordance, kappa 0.64-0.78. Participants preferred the Colli-Pee device over the Evalyn Brush, supporting decentralised, patient-centric screening feasibility.

Among 515 women, 93% believed HPV self-sampling could improve screening uptake and 95% found instructions clear and felt confident, yet 41% still preferred clinician collection and 9-19% found it unpleasant. Two dry vaginal self-sampling devices and a first-void urine collector detected over 90% of high-grade cervical lesions (CIN 3+) and all invasive cancers, with sensitivity comparable to clinician-taken smears. Participants found the devices easy to use and self-sampling was well accepted, which could improve uptake of cervical screening.
