Comparison of central laboratory HbA1c measurements obtained from a capillary collection versus a standard venous whole blood collection in the GRADE and EDIC studies

The finding, in our words

Capillary blood collected by fingerstick showed near-perfect correlation with venous samples for HbA1c (R² = 0.993), with 96.7% of paired results differing by no more than 0.2%. Most participants found the kit easy to use at home, indicating potential for decentralised diabetes monitoring when in-person visits are impractical.

    Capillary blood collection using liquid and dried microsampling devices yielded pharmacokinetic profiles comparable to venous sampling for two monoclonal antibodies and a small molecule, whereas haematocrit correction was necessary for dried formats and bridging was ineffective for hydroxychloroquine. The findings demonstrate the bioanalytical feasibility and patient acceptability of remote microsampling tools for pharmacokinetic evaluations in decentralised clinical trials.

    Capillary blood collected with Tasso+ devices demonstrated strong agreement with venous blood for primary Athlete Biological Passport markers (haemoglobin, reticulocytes, OFF-score, ABPS) with correlations ≥0.9, though platelet count showed poor concordance. Despite 44 of 209 samples failing pre-analytical checks mainly due to coagulation, high athlete acceptance and minimal pain support capillary sampling as a less invasive, viable alternative for decentralised anti-doping controls.

    In a cohort of 62 participants evaluating home self-collection with the TassoPlus device, 38 returned samples, with 29% excluded due to insufficient plasma volume below 200 ΔL. Although viral load measurements in adequate samples correlated well with conventional testing (r = 0.800), the high failure rate and missed detection in low-volume viremic samples indicate that further technical refinements are necessary before clinical adoption.
