The Utility of an Human Papillomavirus Genotype Assay for Cancer Screening in Self-Collected Urine and Vaginal Samples from Japanese Women

In 121 women with abnormal cervical cytology, self-collected vaginal samples detected HPV with 77.8% concordance and 75.7% sensitivity compared to clinician-collected cervical samples using the BD Onclarity assay, while spot urine samples stored under optimal conditions (BD Probe Tec container, <72 h at room temperature) achieved only 63.9% concordance and 46.4% sensitivity. This demonstrates that self-collected vaginal sampling is a viable patient-centric approach for decentralised HPV screening, though urine-based testing requires further methodological refinement to improve clinical performance.

Vaginal self-sampling achieved higher pooled sensitivity and specificity than urine self-sampling for detecting high-risk HPV, making it the preferred option, while urine self-sampling remains a useful non-invasive choice for under-screened populations.
For decentralised testing, a study found dry vaginal self-samples using Copan FLOQSwabs stored up to two weeks at room temperature maintain DNA quality. However, first-void urine collected with the Colli-Pee device may be less sensitive for HPV detection, showing lower positivity and higher cycle threshold values, median Ct 30 versus 27 for other samples.
