Blood phenylalanine concentrations affected peripheral neurotransmitter biomarker concentrations differently in each of nine adults with phenylketonuria. Melatonin increased with tetrahydrobiopterin therapy only when blood phenylalanine fell, indicating that monitoring metabolites in first‑void urine and blood could help optimise individualised treatment.
