Evaluation of accuracy of ambulatory glucose profile in an outpatient setting in children with type 1 diabetes

In children with type 1 diabetes, ambulatory glucose profile showed mean absolute relative difference of 9.56% versus laboratory random glucose and 15.07% versus capillary glucose, with linear regression coefficients of 0.93 and 0.89 respectively, indicating acceptable agreement for outpatient glycaemic monitoring.

    In a head-to-head study of 24 adults with type 1 diabetes, the Dexcom G4 Platinum sensor had a lower overall MARD than the Medtronic Enlite sensor (13.6% vs 16.6% in the clinical research centre and 13.8% vs 17.9% overall), with both devices showing reduced accuracy in the hypoglycaemic range and the Dexcom sensor less affected by this impairment.

    This study validated a method requiring only 10 microlitres of serum and found high agreement between fingerstick capillary samples and venous serum for monitoring twelve antibiotics. The results suggest capillary serum sampling is a feasible, minimally invasive alternative that avoids the haematocrit effect associated with whole blood microsampling.

    In 91 children and adults, capillary C-peptide taken with the TAP device matched venous sampling with 100% sensitivity and specificity for a clinically relevant level, 200 pmol/L or above, with r = 0.996. 63% preferred it to a needle against 7% who preferred the needle, and same-day bruising was 5.5% versus 34.8% after a venous draw.

    Noninvasive salivary measurements of creatinine, cystatin C and urea, and fingerstick capillary blood testing, can approximate gold‑standard laboratory results for kidney function. This could enable more frequent, home‑based monitoring to detect CKD progression and AKI episodes without the burdens of conventional venepuncture and laboratory processing.

    Retrospective analysis of 15,218 paired samples from two paediatric centres showed capillary blood counts differ systematically from venous results, with haemoglobin 6.5 g/L higher and platelet count 7.01 × 10⁹/L lower on average. This large-scale evidence enables accurate interpretation of capillary samples in children, which is crucial for patient-centric microsampling in decentralised diagnostics.
