In adults with sterile pyuria, atypical pathogens were detected in 40% of cases, most commonly Chlamydia trachomatis (19%) and Ureaplasma spp. (15%), with risk factors including multiple sexual partners and past pelvic surgery; comprehensive screening of first-void urine for these agents is warranted.
