Optimisation of a commercial PCR assay for macrolide resistance in Mycoplasmoides genitalium achieved 100% specificity and 90.9% concordance with sequencing. The assay detected resistance in 67% of positive clinical specimens, with comparable performance from first-void urine and swabs, supporting its use for decentralised STI diagnostics and targeted antibiotic therapy.
