First-Void Urine Microbiome in Women with <i>Chlamydia trachomatis</i> Infection

Women with chlamydial infection show reduced microbial diversity in first-void urine, with depletion of Mycoplasmataceae and Ureaplasma parvum and elevated hippurate and lactulose. This demonstrates that urine microbiome and metabolome analysis can reveal infection signatures, supporting decentralised STI screening approaches.

    First-void urine from men with idiopathic urethritis and from controls contains a highly diverse bacterial microbiota with no consistent clustering by case status, although some idiopathic urethritis samples are dominated by single genera previously linked to urethritis such as Gardnerella, Haemophilus or Ureaplasma. The findings suggest that microbiome profiling of self-collected urine may inform aetiological investigation in idiopathic urethritis but requires larger cohorts to define diagnostic utility.

    First-void urine self-collection detected bacterial vaginosis with sensitivity comparable to vaginal swabs when analysed by quantitative PCR for seven key vaginal bacteria. This supports decentralised screening for BV and sexually transmitted infections using patient-collected samples.

    This meta-analysis found that HPV testing using self-collected vaginal and urine samples showed sensitivities of 88.6% and 81% for detecting cervical precancer. Using a standardised device for first-void urine increased sensitivity to 86.6%, supporting decentralised patient-centric screening, though data on specificity and specific commercial devices were not consistently reported.

    A study of 2500 women found 99.0% agreement between first-void urine and vaginal self-sampling for HPV detection using Cobas4800, with 98.1% of participants preferring urine.     This study found that self-collected vaginal swabs and first-void urine yielded comparable detection rates for vaccine-targeted high-risk HPV types, supporting the use of both patient-centric methods for decentralised surveillance.
