Treadmill exercise improves fitness and reduces craving and use of cocaine in individuals with concurrent cocaine and tobacco-use disorder
De La Garza et al.
The finding, in our words
Remote monitoring of heart rate, respiration and locomotor activity distinguished exercise groups from sedentary controls and tracked a significant reduction in resting heart rate after four weeks of treadmill exercise in people with cocaine and tobacco-use disorder. The study also provides preliminary evidence that exercise improved cocaine abstinence, verified both by self-report and biochemical assay.

A fully remote decentralised clinical trial in Danish adults with type 2 diabetes demonstrated operational feasibility with rapid recruitment, high retention (87 percent), and excellent adherence to continuous glucose monitoring (95 percent achieving ≥70 percent data coverage) and telemedicine visits (97 percent). Activity tracker adherence was poor (12 percent) due to technical issues, but remote safety monitoring was effective with no unexpected adverse events, supporting the viability of remote diabetes management.
Older adults with and without cognitive impairment were motivated to participate in longitudinal remote sleep research, finding it acceptable when device burden was low, usability high, and adequate support enabled routine integration. Varying cognitive task content and allowing timing flexibility reduced fatigue, while diverse recruitment remains needed for equitable representation in decentralised dementia trials.

The High-Definition Oncology feasibility study in 30 women with metastatic cancer demonstrated high adherence to biospecimen collection (97.4% plasma, 80.7% stool) and wearable monitoring (70–95% of days captured for activity, heart rate, sleep, and oxygen saturation). This establishes a viable framework for decentralised, multimodal data collection in oncology to support individualised treatment models.

Short-term PM2.5 exposure raised nocturnal cough frequency by 2.4 per cent for each 10 μg/m³ increase across 32 cities. Audio recordings from a mobile sleep app, processed by artificial intelligence, provide a feasible digital biomarker for real-time environmental health surveillance.

Patients with metastatic cancer who recorded ≤3013 steps/day during the 14‑day post‑discharge period had substantially higher rates of 90‑day unplanned readmission (73% versus 13%) and mortality (47% versus 9%) than more active patients. Wearable‑derived mobility is a promising decentralised digital biomarker for risk stratification after hospital discharge, but requires external validation before guiding supportive care.
